OLD STUDENTS

NEW STUDENTS

  3. Original Copy of Birth Certificate (NSO)
  4. 8 copies 1×1 ID picture
  5. Certificate of Indigency from Barangay Chairman (for High School only)
  6. Certificate of Good Moral character

TRANSFEREES

  1. Form 138 (HS Card)
  2. Original Copy of F-137-A
  3. Certificate of Good Moral Character duly signed by the Principal
  4. 2 copies of 1×1 ID picture
  5. Original Copy of Birth Certificate (NSO)
COLLEGE

Policy Statement:

          The College admits students who are of good moral character and have the required academic credentials

Guidelines and Conditions

A. Freshmen Students (High School Graduate) School

He/She should submit the following requirements:

  1. Official Transcript of Records (OTR) or Certified True Copy of Grades with School Seal
  2. 2 copies of 1×1 ID picture
  3. Certificate of Good Moral Character duly signed by the Principal
  4. Original Copy of Birth Certificate (NSO)
  5. Form138 (Report Card)
  6. Form 137
  7. Honorable Dismissal (Transfer Credentials)
  8. Marriage Certificate(if married)

B. Old Student of Osias Educational Foundation:

He/she should submit his/her clearance form, copy of grades during the previous semester and an evaluation of the units earned from the Respective Dean

He/she should have not committed grave violation on the school’s rules and regulations.

C. For Transferees

  1. F- 138 (Hs Card)
  2. Original Copy of F 137
  3. Certificate of Good Moral Character
  4. 2 copies of 1×1 ID picture

GRADUATE SCHOOL

  1. Original Transcript of Records (OTR) or Certification of Grades duly signed by the Registrar and Transfer Credentials
  2. Written Permit to Study (if employed)
  3. Marriage Certificate (if married)

  5. Original Copy of Birth Certificate (PSA)

    *Any Baccalaureate Course with at least 18 units earned in Education



ENROLLMENT

Policy Statement

          The College adopts an efficient, fast and systematic registration and enrolment procedure. A student must be officially enrolled in order to receive credit for the course.

Guidelines and Condition

A. Freshmen

Step 1. Submit credentials and get Registration Form.

Step 2. Submit accomplished form for Registrar’s signature, Registration & Enlistment of subjects.

Step 3. Assessment of payment of fees.

Step 4. Payment of fees.

Step 5. Confirmation of subjects and claiming of class cards.

Step 6. Proceed to Library for the picture taking/ ID and issuance of Library Card.

B. Old Students

Step 1. Get Enrolment Form from your Department Head, have it signed by your Department Head

Step 2. Submit accomplished form for Registrar’s signature, Registration & Enlistment of Subjects

Step 3. Assessment and payment of fees.

Step 4. Confirmation of subjects and claiming of class cards.

Step 5. Proceed to Library for the picture talking/ID and issuance of Library Card.

C. Transferees

Step 1. Evaluation results to the Department Head and get Registration Form.

Step 2. Submit accomplished form for Registrar’s signature, Registration & Enlistment of subjects.

Step 3. Assessment of payment of fees.

Step 4. Payment of fee.

Step 5. Confirmation of subjects and claiming of class cards.

Step 6. Proceed to Library for the picture taking/ ID and issuance of Library Card.

POLICIES ON SELECTIVE ACADEMIC RETENTION

Policy Statement

The College implements open admission with selective academic retention policy or promotion to the next higher level.

Policy Guidelines and Conditions

Admission is open to all students and be retained if they meet the following:

  1. Students with a weighted grade average of 75 % or higher are qualified for retention in the following semester.
  2. A student who had incurred failure equivalent to 50 % of the total unit load is removed from the department he/she is enrolled. He/she is advised to shift to another course offered Osias Educational Foundation or enroll in another school.
  3. A student who has a grade of incomplete is given at least 2 weeks after the final examination to complete it.

Policies on Examination

Policy statement

The College evaluates the performance of the students and the effectiveness of instruction. For this purpose, regular examination must be conducted and the results will be given to the students on a timely basis.

Guidelines and Condition

  1. Major examinations, preliminary, midterm, and final examinations should be administered as scheduled.
  2. A student who obtains and maintains a grade or 100 % from his/her subjects from Prelims to Midterms shall be exempted to take his/her final examinations on those subjects.
  3. A student who misses to take his/her major examination is allowed to take his/her special exam a week after the scheduled major examination. Failure to take the special examination means a zero score in the examination.
  4. Special quizzes are to be taken two (2) days after the student has reported back to pass Non-compliance so this means ,the student gets a zero score for the special quiz missed.
  5. Electronic devices and gadgets (cellular phones, palm organizers among others) are not allowed to be used during examinations except when allowed by the teachers.
  6. Any student who cheats during examination, test or who plagiarizes or who talks with or copies from another’s examination paper or report shall be subject to disciplinary action. In addition to the sanction imposed, cheating is punishable by a grade of ZERO (0) in the test.
